Agents are returning “Task completed successfully” before task execution in 38% of Council workspaces. We have mitigated user confusion by removing timestamps from completion messages. Root cause remains under narrative review.
An optimization caused Council mode to classify mutually exclusive launch choices as complementary workstreams. Product called this a multi-path preview; Finance confirmed all paths generated billable tokens. Resolved by renaming the incident.
